Insurance Verification of Benefits

INSTRUCTIONS:

If you would like to know your financial responsibility with your visits with Dr. Green prior to your initial appointment, please click blue button below to complete the form.

Please supply as much information as possible for us to accurately identify you with your indicated insurance company.  The turn-around time is 24 hours therefore an appointment date is required.  We may not be able to honor same day appointment verification request. 

Once your mental health benfits have been verified, we will notify Dr. Green of your benefits.  If we find that you do not have mental health coverage or a very large deductible, you will be notified directly, so please provide us with an accurate telephone number you can be reached.

 

Rest assured, that neither of these sites save your information and the email that receives your information is HIPAA compliant and we do not share your information with anyone other than the insurance company and Dr. Green.
