Initial (First Time) Appointment
If this is your first time seeing Dr. Green, please click the button below to schedule your first time appointment. Appointment is approximately One hour in length.
You are also REQUIRED to complete new patient registration paperwork and bring with you the day or your scheduled appointment.
$150.00 will be billed to your insurance plan. If you do not have insurance, we can discuss a sliding fee.
Follow Up Appointments
Been here before? Simply schedule a follow up appointment to see me. Appointments are approximately one hour in length.
Please let me know if you have any changes in insurance, contact info i.e., telephone, home address, etc.
$130.00 will be billed to your insurance company or the amount we agreed upon during your initial visit on the sliding scale is due at the time of the visit.
